I can create them but they don't appear.
I enter the values shown (no idea what the page url is though) and nothing happens. I'm assuming I'm missing the page url page...
1: I can't really explain how to do this, I normally sub the default. I can make it for you though if you send me the images you want. For an example of my latest theme:

2: Yes. The place where you uploaded all the FTP files. Go into there and you can edit the text. To edit things like the text in adopt.php, go to lang ==> lang.php

3: The page URL is the extension from your site. So if you say the extension is "test", then your URL to actually view the page will be "http://yourwebsite.com/pages.php?page=test"
That is probably why it won't appear.
